🏥 What is NHSE’s new auto-registration service?

NHS England is rolling out its own auto-registration feature, which connects the Register with a GP surgery service directly to clinical systems.

The NHSE auto registration integrates the Register with a GP surgery service directly with clinical systems.

Important: This is separate from Healthtech-1’s automation of registrations, and they work in very different ways.

  • SystmOne (TPP) practices have the choice to self-enrol from July 2025.

  • Optum (formerly EMIS) are expected to begin a pilot with some practices Summer 2025.


Will all registrations be automated by NHSE?

No, NHSE auto registration currently only applies to a subset of patients (on average around 25%), only if they meet ALL of the following NHS England criteria:

  • Be over 18 years old

  • Use NHS login (with the highest P9 level of ID verification)

  • Be within your practice’s catchment area

  • Be transferring from another GP surgery

  • Not have a sensitive marker on their patient record

➡️ Only patients meeting all of these criteria will be auto-registered through NHSE’s system.


What happens for auto registered patients?

For patients processed by NHSE’s system (not Healthtech-1):

Your clinical system will:

  • Create a task to review/approve a small set of SNOMED codes

  • Save a PDF of the registration form to the patient’s record

You must still manually review and enter:

  • Emergency contact

  • Pharmacy

  • Next of kin

  • BMI

  • Armed Forces status

  • Free-text comments by the patient (e.g. long term conditions info)

⚠️ * The SNOMED codes added through this pilot include some, but not all, of the codes typically applied by Healthtech-1.

Those included by the NHSE auto registration are as follows:

  • smoking status

  • ethnicity

  • carer status

  • weight and height (using the "self-reported" SNOMED codes)

  • some generic codes including "Allergies - mechanism", "Mental symptom finding", "Pre-existing condition", and "Disability"

Alcohol consumption is recorded within the consultation as free text, but not coded.


🔧 How does this affect Healthtech-1 automation?

You have two options on how to proceed, while still using Healthtech-1's automation.

The option you choose will depend on the amount of information you wish to capture on the record, and how much additional work your practice wishes to take on.

Option 1: ❗Use Healthtech-1 for ALL registrations

You choose NOT to enable the NHSE feature.

  • We continue automating 100% of registrations

  • You get the full coding and full dataset

  • Pricing stays exactly the same

Option 2: 🔀 Use NHSE for eligible patients, and Healthtech-1 for the rest

If you enable NHSE auto-registration:

  • NHSE will automatically process ~25% of registrations

  • Healthtech-1 will still automate the remaining ~75%

Pricing

  • You are not charged for registrations processed by NHSE

  • You are charged for registrations processed by Healthtech-1, as usual

But:

The patients processed by NHSE do not receive the full Healthtech-1 coded dataset, so practices will need to:

  • Manually review

  • Manually add missing SNOMED codes if required

Whichever option you choose, we'll still send out the local form to all patients and collect those responses free of charge. You only pay for registrations that we automate.


What about the patients that cannot be auto registered?

If your practice chooses to enable auto registration:

  • Healthtech-1 will continue to fully support all patient registrations outside of the pilot criteria, that is on average around 75% of registrations.

  • All registrations (whether processed by NHSE or Healthtech-1) will still appear in the Hub, including relevant signals.


Will patients still be asked to complete the Local Form?

  • NHSE auto registered patients will still be directed to our Local Form, which will allow you to supplement the auto-registered patients with additional information.

  • The additional information collected in our Local Form will need to be manually actioned for these NHSE-registered patients.


What will practices be charged for?

You’ll only be charged for registrations that Healthtech-1 fully automates.

If you choose option 1 (Healthtech-1 automates all your registrations):

  • ✅ We will continue as before

If you choose option 2 (enabling NHSE automation and Healthtech-1 processes the rest):

  • No charge for NHSE auto registrations by your clinical system supplier

  • Healthtech-1 charges apply only to the remaining ~75% of registrations we still process end-to-end
