What are Signals?
Patients sometimes share something on their registration form that you might want to know about.
We register them on your clinical system as normal, then flag it for you in the Patients to review folder.
We call these notifications 🔮 Signals.
Signals highlight safeguarding concerns, health concerns, and other patient details that your practice may have a follow up action for.
How will I know if a patient has Signals?
If a patient has been registered but they have a Signal, you'll receive this email notification:
Where do I find Signals?
In the Hub, you'll find Signals in the purple section at the top right of a patient's registration form:

What Signals are there?
🖋️ Administrative
The patient has uploaded a document (eg. a baby red book or vaccination record)
We were unable to update the Pharmacy preferences for this patient. Please add this
We were unable to update the Summary Care Record (SCR) preferences for this patient. Please add this
We were unable to update the systmOne Record Sharing preferences for this patient. Please add this
The patient has a European Health Insurance Card (EHIC)
This patient has stated that they have a firearms licence*
This patient has stated that a member of their household has a firearms licence*
👥 Demographic
The patient has a social worker
The patient is fostered
The patient has a disability
The patient is out of area*
The patient is a carer
This patient has a main spoken language which has not been coded*
Patient's gender identity has changed since sex at birth*
This patient's stated gender is In Another Way*
🚑 Medical
The patient says they have repeat medications
The patient says they have long term conditions
The patient says they are pregnant*
The patient says they are pregnant and not booked in with maternity services*
The patient says they have a contraceptive device/implant and have provided a date it is due to be removed on*
The patient would like a HIV test*
The patient would like a chlamydia test*
The patient says they have special circumstances (Approaching end of life, Asylum seeker, experiencing homelessness, Housebound, Refugee, Victim of domestic abuse)*
The patient has a high alcohol FAST score (8+)
The patient has a high alcohol AUDIT-C score (10+)
The patient says they have mental health conditions
The patient says they require reasonable adjustments
The patient says they need to be communicated with in an accessible format
The patient has free texted an allergy which has not been coded
The patient may have missing vaccinations
The patient had vaccinations abroad
The patient may be eligible for TB Screening
The patient has a low/high weight/height which has not been coded
This patient is eligible for a Hepatitis C blood test
This patient has stated that they require regular or ongoing support for their mental health or emotional wellbeing*
🛡️ Safeguarding
The patient is a child (under 18 years old)
The patient is under 16 years old and their stated person of parental responsibility is anyone other than their mother or father
This patient has a carer which we have not added to the patient's record. Please add this
This patient has a second carer which we have not added to the patient's record. Please add this
The patient has professional involvement (Hospital specialist or consultant, Health visitor, Social worker, Other health professional)
This patient receives support from a professional agency*
* These Signals are part of configurable Local Form questions which may not feature on your practice's registration form. You can toggle these on by going to your Local Form settings (NHSE national registration form):
Can we reduce the number of signals we get?
Yes! Signals are optional.
If there's a Signal your practice doesn't need, head to Settings > Signal settings and switch it off - then you'll only see what's useful to you.
