An overview of how patient registrations work in the Hub – how patients submit their form, where to find registrations, and what each folder means.

How do patients fill out the registration form?

Patients access the registration form through your practice website, the NHS Find a GP website, or your practice QR code – whichever they use, it's the same NHS national registration form.

Once patients have submitted their registration form, they will be asked to fill out a second optional Local Form which we have created to capture additional information for practices that can be customised to your practice needs.


What happens after a patient has filled out a form?

After a patient has completed their form, it gets sent to us and lands in Patients to Register > Pending automation – our processing queue in the Hub. Forms are completed within 48 working hours, though the vast majority are processed between 6–10 hours!

Since 5 December 2025, once we have successfully automated a registration, we automatically save a PDF of the registration form directly into the patient's clinical record.


Where will I find a registration form in the Hub?

Depending on the status of the registration, it will be found in one of four folders:

  • Patients to Register > Pending automation – form received and currently being processed by us
    ​

  • Patients to Register > Automation unsuccessful – we were unable to automate the registration, we may need you to confirm some details so we can complete the automation
    ​

  • Patients to Review – registration is complete but signals have been flagged for your awareness
    ​

  • Archived – registration is fully complete with no further action required, accessible for 60 days before redaction


How do I find a specific patient?

To find a specific patient's form, click Find a Patient at the top of the Hub. You can search by date of birth, last name, or NHS number.


How do I read a registration form?

To read a registration form, click on a patient's name and their form will open up and look a little something like this πŸ‘‡

  1. Form responses: the sections in the white boxes on the left and middle show you the patient's answers to each question on the registration form, as well as any Local Form answers.

  2. Signals: the purple section highlights key information from the patient's responses that we think may be of interest to you. You can turn off any signals your team doesn't need.

  3. Admin notes: the yellow section on the right tells you what's happening with a patient's registration. Required actions will show up here, as well as any updates you and your colleagues leave for each other.
