The Local Form is an optional form created by HT1 that allows you to ask tailored questions to meet local incentive needs and serve your new patients. But wait, there's more! You can now request your own questions too - more details on how to do that below 😁.

What is the Local Form?

We collaborated with NHS E to enhance the national solution by offering a local form that puts you back in control!

Automatically activated for practices at no additional cost, this form builds on the standardised national registration form to capture and code additional data, including health inequalities, special circumstances, and screening data relevant to your practice, all aimed at enhancing patient care.

What questions do you ask?

You can check out the full list of questions here. Feel free to turn any of these questions on or off as you like under 'Settings' > 'Local Form Settings' on the Hub.

Can I request a new question?

If you’ve got an idea for a new Local Form question that would be helpful for practices, just use the button below to submit it! We review submissions regularly and add new questions periodically. πŸ‘‡

How does the local form work?

Once a patient finishes completing the NHS E national registration form, they'll be directed to complete the local form. This is their chance to share a bit more by answering some extra questions from the practice, helping you get a clearer picture of their health needs.
