What is the process for registering a patient gender reassignment?

Patients aged 18 and over can request a gender change on their medical record at any time. They do not need to have undergone any gender-affirming treatment.

The PCSE process requires that they usually are given a new NHS number and would be registered as a new patient at your practice. All previous medical information relating to the patient needs to be transferred into a newly created medical record, blanking out their previous NHS number and gender.

Alternatively, a patient may change their gender marker without changing their NHS number. This must be discussed with the patient, and it is advisable for them to sign a simple confirmation form. This form should be added to their record, as earlier records will still show their birth gender.
