This article explains how the emergency contact question appears on the NHS England National Registration Form and how we record these responses in your clinical system.

As part of the NHS England National Registration Form, patients are asked whether they have an emergency contact - someone the practice can contact in an emergency if the patient can't be reached.


What are patients asked?

The form asks: "Do you have an emergency contact?" - patients simply select Yes or No.

If a patient answers โ€œYesโ€

If a patient selects Yes, they are asked to provide:

  • Full name of the emergency contact

  • Relationship to the patient

  • Contact telephone number (international numbers accepted with country code)

  • Optional checkbox: "This is my next of kin (NOK)"

By continuing, the patient confirms that your practice can contact this person in an emergency.

Where does the information get recorded?
