🌎 Patients coming from abroad

These are the questions asked to patients on the NHSE national form regarding previous countries lived in and moving to the UK 🗺️.

As part of the NHS England National Registration Form, patients are asked a number of questions about GP registration history, moving to the UK, country of birth, and language or interpreter needs.

The form includes the following questions:

1. Registering with a UK GP

The form includes the question:

“Are you registering with a UK GP for the first time?”

Patients can select Yes or No.

2. Moving to the UK

The form includes the question:

“Have you recently moved to the UK from another country?”

Patients can select Yes or No.

3. Country of birth

The form includes the question:

“Where were you born?”

Patients are asked to enter their country of birth and town or city nearest to where they were born.

4. Language interpreter

The form includes the question:

"Do you need a language interpreter?"

Patients can select Yes or No.

If a patient selects “Yes” to "Do you need a language interpreter?" they are then asked:

“Which language do you need an interpreter for?”

Patients are shown a list of languages to choose from.

  • If their required language is listed, they can select it.

  • If their required language is not listed, they can select “Other” and free-type the language they need.

‼️ Important note

By default, the language interpreter question is not enabled on the NHS England National Registration Form.


5. Entering the UK

The form includes the question:

“When did you enter the UK?”

Patients are asked to provide a date.

6. Moving from the EU, EEA, or Switzerland

The form includes the question:

“Have you moved to the UK from the EU, EEA, or Switzerland?”

Patients can select Yes or No.

If a patient selects “Yes”, they are asked whether they have any of the relevant documents.

  • European health insurance card (EHIC)

  • S1 form

  • I don't have any of these documents.
