These are the questions asked to patients on the NHSE national form regarding previous countries lived in and moving to the UK πŸ—ΊοΈ

1. Are you registering with a UK GP for the first time?

2. Have you recently moved to the UK from another country?

3. Where were you born?

4. Do you need a language interpreter? If yes, for which language?

‼️ By default this question is not turned on for the NHSE national form. NHSE requires the GP practice to turn this question on. You can manage what questions you ask patients in NHS Profile Manager.

5. When did you enter the UK?

6. Have you moved to the UK from EU, EEA or Switzerland? If yes, do you have any of these documents?
