These additional questions relate to extra services that some practices offer, and you can choose whether or not to include them in the registration process.

Some questions are not included by default on the NHSE national form. To enable these questions, practices need to access the NHS Profile Manager and manually turn them on.

​1. πŸ’Š Direct Dispensing (for dispensing surgeries) ​

If you are a dispensing practice, you can ask patients if they wish to receive their medicines or medical aids directly from your surgery.

Patients must meet specific eligibility criteria, such as:

  • Want to get their medicines or medical aids direct from your surgery

  • Live more than 1 mile from their nearest pharmacy, or

  • Would have serious difficulty getting to their nearest pharmacy

Based on their answers, the system will inform them:

  • If they are not eligible for the service

  • If they may be eligible and that you might discuss it with them further

We follow the Dispensing Catchment Area rules: Patients living more than 1 mile (1.6km) away from a pharmacy will be a dispensing patient.


2. πŸ—£οΈ Interpreting Services ​

If you’d like to offer interpreting services, you can ask patients if they need a language interpreter for their appointments.

The service will ask the patient:

  • If they need an interpreter.

  • Which language they need an interpreter for


3. πŸ₯ Nominating a Pharmacy​

You can ask patients if they would like to nominate a pharmacy for their prescriptions.


If they answer yes, they will be asked:

  • If they prefer an online or high street pharmacy

  • To search for and select a high street pharmacy using a postcode look up, or

  • To register directly with an online pharmacy


4. πŸ‘©β€πŸŽ“ Identifying University Students​

If your practice is university-affiliated or you register many university students, you can ask students if they are currently enrolled at a UK university.


The patient will be asked:

  • If they are a student at a UK university

  • For their student identification number

  • The year their course finishes

  • Where they are living while at university


5. πŸ’‰ Tuberculosis (TB) Screening​

You can offer tuberculosis (TB) screening to patients, especially those who may be at higher risk.

This question identifies patients who:

  • Are aged between 16 and 35.

  • Have recently arrived in the UK from a non-EU country

  • Were born in or have spent over 6 months in a country with a higher risk of TB

For eligible patients, we will leave a signal on their registration form indicating they may be eligible for TB screening.
