What questions does the NHSE national form asks specifically when registering children?
When registering a child on the NHSE national form, the following questions are asked:
What is your relationship to the person being registered? (parent or guardian, carer, other)
Are you currently registered or have you submitted a registration?
Will you be registering yourself?
Do you know your NHS number?
What are your details? (name, relationship, date of birth, contact number)
If the patient is less than 12 months old:
Where was the child born?
Enter the postcode where the mother of the child was living when their baby was born
If the patient is less than 18 years old:
Does the child go to any of the following for their education?
Is anyone else involved in the child's care?
Has the child had all their routine vaccinations?
Did the child get all their routine vaccinations in the UK?
Do you check the person with parental responsibility is registered at the practice?
The NHSE national form asks whether the child who is getting registered has a person with parental responsibility (PPR) who is already registered with the practice and if they are not, the below notification appears.

On our end, for all children under 16, the PPR must also be registered with the practice for our automation to process the registration.

โ The registration will not be automated if the child is registered without a parent who is a patient at the practice.

โ If the child does have a registered PPR at the practice, the system will assign the child's Usual GP to match that PPR.
Do you ask for the Baby Red Book and vaccination records?
We ask guardians of children between 8 weeks and 6 years to upload the Baby Red Book onto the Hub.

This document is not automatically uploaded onto the patient record, but you are able to access and download this from the Hub.

When this is uploaded, you'll see a signal for documents uploaded and you'll be able to download the documents from the patient's registration form.

Child has a social worker
Child is fostered
If the person of stated parental responsibility is not the mother or father
We also have signals that are the same for adults and children which are:
Patient has long term conditions
Patient is on repeat medication
Patient has a disability
Patient is out of area (if applicable)
How do you automate registrations for babies who don't yet have a name?
We can automate patients who have the name of 'Baby' but will sometimes flag them to you or ask you to double-check their details ๐ฅ
