Signals highlight important information patients put on their registration form which you should review.

What are Signals?

In some cases, patients are registered automatically but they enter important information in their registration form which might require a follow up action.

We call these notifications ๐Ÿ”ฎ Signals.

Signals highlight safeguarding concerns, health concerns, and other patient details that are important for you to make note of.

How will I know if a patient has Signals?

If a patient has been registered but they have a Signal, you'll receive this email notification:

Where do I find Signals?

In the Hub, you'll find Signals in the purple section at the top right of a patient's registration form:

What Signals are there?

๐Ÿ–‹๏ธ Administrative

  • The patient has uploaded a document (eg. a baby red book or vaccination record)

  • We were unable to update the Pharmacy preferences for this patient. Please add this

  • We were unable to update the Summary Care Record (SCR) preferences for this patient. Please add this

  • We were unable to update the systmOne Record Sharing preferences for this patient. Please add this

  • The patient has a European Health Insurance Card (EHIC)

๐Ÿ‘ฅ Demographic

  • The patient has a social worker

  • The patient is fostered

  • The patient has a disability

  • The patient identifies as non-binary

  • The patient describes their gender in another way

  • The patient says their current sex is different from their sex at birth

  • The patient is out of area*

  • The patient are a carer

๐Ÿš‘ Medical

  • The patient would like Blood Borne Virus (BBV) screening*

  • The patient says they have repeat medications*

  • The patient says they have long term conditions*

  • The patient would like a HIV test*

  • The patient would like a chlamydia test*

  • The patient says they have special circumstances*

  • The patient has a high alcohol FAST score (8+)

  • The patient has a high alcohol AUDIT-C score (10+)

  • The patient says they have mental health conditions

  • The patient says they require reasonable adjustments

  • The patient says they need to be communicated with in an accessible format

  • The patient has free texted an allergy

  • The patient may have missing vaccinations

  • The patient had vaccinations abroad

  • The patient may be eligible for TB Screening

๐Ÿ‘ถ Safeguarding

  • The patient is a child (under 18 years old)

  • The patient is under 16 years old and their stated person of parental responsibility is anyone other than their mother or father

  • The patient is under 16 years old and does not live with their stated person of parental responsibility

  • This patient has a carer which we have not added to the patient's record. Please add this

  • This patient has a second carer which we have not added to the patient's record. Please add this

  • The patient has professional involvement (Hospital specialist or consultant, Health visitor, Social worker, Other health professional)

* These Signals are part of configurable questions which may not feature on your practice's registration form. You can toggle these on by going to your form settings:

Can we reduce the number of signals we get?

Yes! You can turn signals off if you're finding they're not useful for your team. To do this, send us a chat or email and we'll toggle them off.
