How long do registration forms stay on the Hub?
In line with our Data Processing Agreement with NHS England, registration forms remain accessible on the Hub for 60 days. This gives you time to refer back to them if needed.
What happens after 60 days?
After 60 days, all patient-identifiable data is securely scrubbed from the registration form.
The form will still be visible in the Archived folder on the Hub, but you won’t see any identifiable patient information 👇
Where else can I find the form?
NHS England automatically emails a copy of each completed registration form to your shared practice inbox as soon as a patient submits it.
If you're looking for a form that's older than 60 days, we recommend checking that inbox.

Do you save a PDF copy of the form into the patient notes?
Yes, we do now save a PDF copy of the form into the patients notes! ✨
How to download a copy of the form (before it’s redacted):
Use the Find a patient feature to locate the registration form
Open the form and click ‘Download as PDF’ in the top right corner
Done – the file will appear in your downloads folder!
