Do you ask patients if they have served in the Armed forces?

Yes! We've worked with a number of practices to ensure that the questions we ask align with the requirements of the Veteran Friendly Accreditation.

This gets a little more complicated as it involves questions that NHS England ask, and questions that we ask as part of the Local Form.

On the NHS England registration form, patients are asked:

If a patient answers 'Yes' to this, when they go on to complete the local form they are asked:

  • If you have previously been registered with a UK Armed Forces GP, what role were you registered as?

  • What is your enlistment date to the Armed Forces?

  • What date did you or will you leave the Armed Forces?
