Do you ask patients if they have served in the Armed forces?

Yes, this involves questions that NHS England ask on the national form, and questions that we ask as part of the Local Form, that align with the requirements of the Veteran Friendly Accreditation.

On the NHS England registration form, patients are asked:

If a patient answers 'Yes' to this, they are shown the below on the NHSE National form:

And then when they go on to complete the local form they are asked:

  • If you have previously been registered with a UK Armed Forces GP, what role were you registered as?

  • What is your enlistment date to the Armed Forces?

  • What date did you or will you leave the Armed Forces?
