Describes the Local Form special circumstances question and how patient selections like homeless, housebound, or refugee are recorded in their record.

The Local Form is a separate follow-on form that allows patients to provide extra information after completing the NHS England registration form.

Special circumstances is one of several local form questions and captures situations that may affect a patient’s care.


What are the special circumstances options?

Patients can select one or more of the following options:

  • Approaching end of life

  • Asylum seeker

  • Experiencing homelessness

  • Housebound

  • Refugee

  • Victim of domestic abuse

How do I make this question available?

To make the Special Circumstances question visible to patients:

  1. Go to the ⚙️ Settings page of the Hub.

  2. Scroll to Local Form settings on the left-hand side.

  3. Tick the Special Circumstances question.

  4. Scroll to the bottom and click Save Settings.

Which SNOMED codes are added?

The following SNOMED codes are recorded in the patient’s record for the corresponding options:
