On the NHS England registration form, patient's are asked the AUDIT-C alcohol questionnaire.

How often do you drink alcohol or have a drink containing alcohol?

How many units of alcohol do you drink on a typical day when you are drinking?

How often have you had six or more units of alcohol on a single occasion in the last year?

Do you code alcohol?

Yes, we do, to make sure you hit QOF. Here are the codes we use depending on the patient stated answers:

Current drinker, 219006

Non - drinker, 105542008

Alcohol units per typical day of drinking, 443315005

Declines to state current alcohol consumption, 858861000000101

AUDIT-C (Alcohol Use Disorders Identification Test - Consumption) score, 763256006

Do you signal alcohol?

The total score for AUDIT-C is between 0-12. If a patient has a score greater than or equal to 10 then we will ๐Ÿ”ฎSignal this.

What about the full AUDIT questionnaire?

Currently the NHS England registration form just asks patient's three questions to calculate the AUDIT-C score. The full AUDIT score consists of 10 questions, with a total score between 0-40.

There are currently no plans for us to add these further questions to the Local Form.
