What are special circumstances?

Patients will be able to select from the following options:

  • Approaching end of life

  • Asylum seeker

  • Experiencing homelessness

  • Housebound

  • Refugee

  • Victim of domestic abuse

How do I make this question available?

You can turn this question on by going to the โš™๏ธ Settings page of the Hub, under 'local form questions'

Make sure you scroll to the bottom and click 'save settings' once you've configured the local form questions you want to have.

What will patients see on the form?

Which SNOMED codes are added?

  • Approaching end of life - due to the sensitivity and impact of this, we do not code it. Please take the appropriate post registration action via the Signals section

  • Asylum seeker, 390790000

  • Homeless, 160700001

  • Housebound, 160689007

  • Refugee, 446654005

  • Victim of domestic abuse, 879911000000102

Anything else?

We will add a ๐Ÿ”ฎ Signal so that you can review and complete any post registration actions.
