How to add optional questions on the NHS England registration form

These additional questions relate to extra services that some practices offer, and you can choose whether or not to include them.

Some questions are not included by default on the NHSE national form. To enable these questions, practices need to access the NHS Profile Manager and manually turn them on to suit your surgery.

At our practice in Stratford, we have the following settings:

  • Language interpretation: Yes

  • Dispensing surgery: No

  • Registering students: No

  • Nominated pharmacy: Yes

  • Tuberculosis screening: No

To adjust these, sign in to the NHS profile Manager πŸ”—

Click "Sign up to the Register with the GP surgery service" instead of "Manage Profiles".

Then click "View details" under your surgery. If you don't see any registered surgeries here, check out this article.

Then click "Change" under the additional services area.

You can then adjust and save your settings. For example, if you mostly register university students, it's helpful to ask for additional information on whether the patient is a student.

We can set up different automations depending on the fields on the registration form. In the example above, by turning on student questions, we can set up an automation to assign all registered students to a single branch practice. Magic huh ✨
