What question do you ask about family history?

We ask the patient if they have any family history of certain conditions, and code the information in the patients record.

Conditions we ask about include: asthma, COPD, epilepsy, heart disease, stroke, high blood pressure, diabetes, kidney disease, liver disease, depression, thyroid and cancer.
